The first combination lock?

Second-hand door? Or there are two key holders because inside is the launch button for Gloucestershire's intercontinental ballistic missile, targeted on Rockall.

That would be two key holders with extremely short arms then, Dave.

Perhaps it was the door to a secret society of some kind, and it took two people (each with a different key) to unlock the door and gain entrance.

Inhabitant gave Significant Other a key. Significant Other become Insignificant Other. Local locksmith couldn't change the lock so added new one. just underneath the shot there are 4 other keyholes ...
